You may have noticed a growing trend in the telehealth space: some providers are offering additional peptides alongside GLP-1 medications. New names such as NAD+, sermorelin or ipamorelin are often presented as potential complements to GLP-1 therapy.

At the same time, I've been noticing more and more people sharing their experiences with these combinations on social media. Sure, some posts are probably influenced by marketing, but they also show that some people in the GLP-1 community are curious and have real questions about whether these add-ons are actually worth it.

Today I'll look at what science currently knows about these peptides and whether they can actually support GLP-1 therapy.

BPC-157

BPC-157 is a lab-made peptide that was designed to mimic a small piece of a protein found in stomach fluid. People take it to fix nagging joint pain, repair torn tendons and ligaments, and even soothe gut issues.

How it connects to GLP-1 therapy: The link isn't pharmacological — GLP-1 drugs and BPC-157 don't interact directly. The link is behavioral. Once appetite and weight start coming down on weight loss medications, some people begin feeling better and become more active - taking long walks they couldn't manage before, or even getting into running or lifting. Then suddenly, an old injury can flare up, or a new overuse issue can appear because the body isn't used to the increased load yet. That's often when people start searching for solutions and come across threads full of stories about faster recovery from shoulder problems, back pain, or old sports injuries.

I've read through Facebook groups and patient reports, and the stories usually focus on things like faster bruise healing, less tendon pain, and joint function that feels "better than baseline."

What we actually know: The good news is that in animal studies, particularly in rats, BPC-157 has shown very promising results. It appears to help promote the formation of new blood vessels and significantly speed up the healing of tissues like Achilles tendons and ligaments.

The catch is that human evidence is still extremely limited. While the biological mechanisms look interesting in laboratory studies, we don't yet have large, well-controlled human trials showing that the same benefits apply to people.

NAD+

NAD+ is a crucial molecule found in every cell of your body. It acts like a spark plug for your mitochondria, helping convert food into energy and repairing damaged DNA. In the context of weight loss and weight management, it converts food into energy.

How it connects to GLP-1 therapy: With appetite suppression and fewer calories coming in, some people start to feel drained - low energy, brain fog, that "running on empty" feeling. Since NAD+ plays a key role in how our cells produce energy, it can seem like a natural thing for tired GLP-1 users to look into, especially when basics like hydration, enough protein, and good sleep aren't fully fixing the fatigue.

Anecdotally, some users say they notice a clearer head and less brain fog after NAD+ drips, but many also point out that the effects can be modest and the cost can add up quickly.

Overall, the community sentiment actually tracks the science fairly well here. Most people treat NAD support as a possible marginal add-on to basics like hydration, protein intake, sleep, and getting GLP-1 dose titration right.

What we actually know: Human trials show that oral NMN and NR reliably increase blood NAD+ and produce modest improvements in insulin sensitivity, muscle function, or fatigue scores.

Good mechanistic case and decent biomarker evidence, but the long-term outcome data isn't there yet.

Sermorelin

Sermorelin doesn't put growth hormone into your body directly — it tells your own pituitary gland to release more of its own. It's a lab-made copy of a small piece of GHRH, the natural signal your brain sends when it wants the pituitary to make growth hormone. That's the whole pitch behind calling it more natural than injecting growth hormone itself: it works with your existing hormone pathway rather than replacing it.

How it connects to GLP-1 therapy: People usually start looking into it after hitting a weight loss plateau, feeling softer or less strong than expected. They are hoping that increasing growth hormone levels will help them hold on to muscle while losing weight and improve sleep and recovery. The idea makes sense because growth hormones are naturally involved in muscle maintenance and tissue repair.

What we actually know: Sermorelin can increase growth hormone levels and has a legitimate medical history, mainly in people with growth hormone deficiency and in pediatric medicine. In those cases, it has been used as a real clinical tool.

The question is whether those same effects translate to people using GLP-1 injections who are hoping for muscle preservation. The idea that sermorelin is a safer or more natural option because it stimulates the body’s own hormone production makes sense in theory, but it hasn’t been proven through clinical trials.

Tesamorelin

Tesamorelin is a longer-acting cousin of sermorelin — same basic idea of prompting your pituitary to release growth hormone, but engineered to stay active longer. What sets it apart from everything else on this list is that it's actually FDA-approved, specifically for reducing excess deep abdominal visceral fat in people with HIV-associated lipodystrophy, a fat-redistribution condition. It's one of the only compounds here with real, large-scale human trial data behind it.

How it connects to GLP-1 therapy: Because tesamorelin has actual randomized trial evidence for shrinking visceral fat, some people on GLP-1s look at it when their goal isn't just lose weight but specifically reshape body composition — targeting stubborn abdominal fat while trying to hold onto muscle and a leaner overall look. That's a more specific goal than a lower number on the scale, and it's why tesamorelin shows up in body-recomposition conversations more than in general weight-loss ones.

What we actually know: In a 12-month randomized placebo-controlled trial of people with HIV and central fat accumulation, tesamorelin reduced visceral fat by about 11% after 6 months and by approximately 18% after 12 months in those who remained on treatment.

More recent pooled RCT data also support reductions in visceral fat, trunk fat, and liver fat, along with an increase in lean body mass, with generally tolerable adverse effects such as arthralgia, myalgia, paresthesia, and injection-site reactions.

Ipamorelin & CJC-1295

This is the most famous duo in longevity clinics. CJC-1295 tells the brain to release growth hormone over a long period, while Ipamorelin triggers quick, powerful bursts of it.

How it connects to GLP-1 therapy: This stack shows up in the same GLP-1 conversations as sermorelin and BPC-157 — recovery, sleep, and preserving lean mass during rapid weight loss. Reddit users report improved gym performance and less flatness.

What we actually know: JC-1295 can increase growth hormone and IGF-1 levels for several days after a single dose in healthy adults. Ipamorelin appears to be generally well tolerated in short-term studies and has also been studied for effects on gut movement.

There are no good human trials showing that taking CJC-1295 and Ipamorelin together leads to meaningful changes in fat loss, muscle gain, body composition, or physical performance. We also don’t have long-term safety data on using this combination.

Where This Leaves Us

If you zoom out, there's a pattern: strong preclinical or mechanistic stories and thinner human outcome data. Tesamorelin is the outlier with the strongest human RCT evidence, but even that evidence is for a specific condition, not general fat loss or GLP-1 stacking.

There's also a broader pattern in why these peptides keep coming up specifically in GLP-1 communities: rapid weight loss creates side effects and new goals — fatigue, muscle loss, sudden activity that injures untrained joints, a desire to fine-tune body composition — and each peptide above gets pulled in to solve one of those specific problems.

None of that means these compounds are useless. It means the evidence is earlier-stage, narrower or more indication-specific than the marketing usually suggests.
